Tournament being hosted by Kitchener Fastball Promotions starting July

select "Application for OASA Affiliation" either "doc" or "pdf" to download 
an affiliation form.  Please forward the completed form with your cheque 
for the applicable fee to the OASA Registrar as identified on the form.
